Your client ticks every one they’re getting today: microblading, powder or ombré brows, lip blush, lip liner, eyeliner, areola, scalp micropigmentation, scar camouflage, beauty marks, or a correction of earlier work.

What this visit is
A first session, a perfecting visit, a color refresh, or work done somewhere else. Every visit is its own signed waiver.

The health questions permanent makeup asks
Accutane in the past 12 months, Botox or filler near the area, peels, laser and retinoids, keloids, and earlier work in the same spot. Cold sores for lips. Contacts, lash extensions and eye drops for eyeliner. Surgery or radiation for areola and scar work. Each question shows only with the procedures it belongs to.

What your client acknowledges
It looks darker at first, then softens. It fades, and most people need a perfecting visit 4 to 8 weeks later. They saw the shape and the color and approved both before you started. And the rest permanent makeup needs said, down to telling the technologist before an MRI.

- Lot numbers for the needle and the pigment on the record, when you turn them on

Do I need an iPad at the counter?
No. Your client signs on their own phone from the QR code on your counter. Nothing to install.
Can my clients sign in Spanish?
Yes. The cosmetic form is in English and Spanish, and your client picks.
What if I tattoo or pierce too?
Say so in Settings once you’re in. Your client then picks what they’re here for when they sign, and gets that form: a cosmetic tattoo, a tattoo or a piercing.
Can a client under 18 sign?
Not for a cosmetic tattoo. The form takes clients 18 and over for one, in every state.
Does it follow my state’s rules?
It asks your state’s own questions and shows its notices, as a tattoo form does. Where a state has a rule of its own for cosmetic work, the form follows it where a form can: in Georgia, no eyeliner within an inch of the eye. Your state’s page on this site says what the form carries there and what stays on paper.
